    I hereby declare that I have read the Club's Terms and Conditions and express my willingness to participate in Brazilian Jiu-Jitsu, MMA, Self-Defense, Kick Boxing, Wrestling classes organized by Rio Grappling Club based in Wrocław. I also declare that there are no contraindications to my participation in classes organized by Rio Grappling Club. I acknowledge the risk of injury, and in the event of its occurrence, I will not seek compensation or damages from the organizer.

    I undertake to immediately provide a medical certificate confirming my fitness to participate in such activities.
